Infection Prevention Control - Are you or have you been suffering from diarrhoea and / or vomiting in the last 48 hours?

If you have please DO NOT come in to the surgery, By doing so you will pass the virus onto staff and other service users. Stay at home. Drink plenty of water to avoid dehydration and let the illness run it's course - usually 3 days. For advice about self-care visit www.nhs.uk  If you are worried then call the GP surgery or NHS 111.

Cancelling your Appointment

Appointments are often in short supply so please ring and let us know if you cannot attend. It means that the appointment can be given to somebody else.

All new patients are offered a check-up on joining the practice and this service is also offered to all patients between 16 and 75 years.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will need to visit the surgery where you will be asked to sign the form to confirm that the details are correct.  The registration process cannot be completed without a signature on the form.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
